S.S. Lomis / Natakhtari Brewery announces a tender for: Employee Health Insurance.

Basic Insurance Conditions: 1) Full health insurance, which must include three categories of insurance packages (involving three different value packages for different levels of employees) and the following minimum conditions: - Emergency hospital services: 100% - Hospital services in case of accidents: 100% - Planned hospital services: 100% - Oncology: 100% - Interventional cardiology / stenting / cardiac surgery: 100% - Emergency outpatient services: 100% - Planned outpatient services with a personal doctor's referral from 90% to 100% (depending on the insurance package category) - Planned outpatient services without a personal doctor's referral: from 10% to 100% (depending on the insurance package category) - Prescribed medications: from 50% to 100% (depending on the insurance package category) - Outpatient services provided abroad: from 0% to 100% (depending on the insurance package category) - Preventive examinations: 2 times a year / 100% - Pregnancy / childbirth: 100% - Emergency dental care: unlimited / 100% - Planned dental services in provider clinics: from 90% to 100% (depending on the insurance package category) - Emergency / medical assistance: 100% - Personal doctor services: 100% - Personal doctor services in Natakhtari: at least 2 days a week - Personal manager services: 100% - Travel insurance.

2) Health insurance for family members (spouse and children) considering the above conditions and additional family members (siblings, parents) health insurance (if different limits apply to additional family members, please provide a detailed list).

3) Reimbursement of treatment and medication costs for pre-existing diseases and conditions (including pregnancy and childbirth), their complications, without a waiting period for insured persons.

Our main requirements are: - A table of insurance company provider clinics in Tbilisi and regions. - A detailed explanation of exceptions (a specific table of diseases and their groups not covered by insurance, without any "etc."). - If the insurance company does not have a dental provider in a specific region/city, the insured must have the possibility to use a free choice scheme: (payment -> reimbursement). - Insured individuals must have the right to contact any licensed doctor, medical institution, pharmacy; a description of the scheme for receiving services in non-provider clinics (including a table of necessary documents to be submitted by the patient). - It is possible to consider several product options, price and product compliance, for both basic and additional family member products.

The number of people to be insured currently is 925 people (including family members of the insured).

Additional conditions and comments: - Due to the company's activities, it is necessary to fulfill the following conditions: - Pre-existing disease reimbursement for all insured individuals, without a waiting period; - Free scheme for using insurance services; - Reimbursement of costs for chronic disease diagnosis, control studies, and consultations; - Reimbursement of costs for examinations and treatment of heart and blood vessel diseases; - Surgical and therapeutic treatment of oncological diseases; including chemotherapy and radiation therapy; - Reimbursement of the value of purchased medications; - Insurance financing regardless of any state program's validity; - Possibility of changing the insurance package; - Children up to at least 21 years old.

Interested parties must submit: - A detailed list of health insurance services and prices; - An extract from the business register; - A draft contract to be concluded (with the following information: conditions for reimbursement of medical expenses, necessary documents, and procedure; a detailed list of exceptions); - A list of providers; - Additional/different offers, if any.
